ALEs (50 or more full-time plus full time equivalent employees) must provide each full time employee with a Form 1095-C. All employers of any size who offer Minimum Essential Cover under self-funded plan generally must provide a Form 1095-B to all covered employees and former employees that describe the health coverage.
